Pus Spitter:
Originally created by a powerful cleric of Nerull with a sick sense of humor, the pus spitter is a virulent green-colored heavy crossbow that feels like it's covered in a slimy mold and looks as if the wood it's made of is slightly rotted. It is a +4 acid heavy crossbow that forces an enemy to make a fortitude save (DC 17) or become nauseus, suffering a -2 penalty to all attack rolls, weapon damage rolls, saving throws, skill checks, and ability checks for 1d6+2 rounds.

Venom Ward:
The venom ward is a powerful tool against cowardly enemies who rely on hiding in shadows or poison to get the drop on them. It is a glittering gold in color and never seems to get dirty. It funcions as a +3 breastplate that sheds light in a 20ft. radius and offers a +4 luck bonus to saving throws against poison.

Hellplague:
The first of these tools of evil were last held by the Gelugon leutenant called Hath-Rulnex. It is a straight, very well made +3 flaming longsword of venom that adds +2 to the DC of any fire spell cast by the wielder. Nonevil creatures suffer 1 negative level as long as the sword is held or used. The poison DC is 17.

Kinemil's Awl:
Kinemil was a paladin of Heronius that made it his cause to slay every demon he came across. Unfourtunately for him, his zeal caused him to slay a local gnome who pranked him with a demonic-looking costume. Because of his contributions, he was let off rather easily, but he felt not so. He rammed the sword into the ground and ran, exiling himself from the world. The sword was given to city hall, to be stolen the following week. His tool was a large golden blade with wings at the hilt, being used as a +3 flaming greatsword that was imbued with the power of a constant bless weapon spell. Any evil character that weilds Kinemil's Awl gains 1 negative level until they stop using or holding it.

The Salamander:
The salamander is a badge of power among the Savants of the Flame. It's incredibly expensive, but just as potent. It is a +3 flaming quarterstaff that imparts fire resistance 30 upon the weilder. It also raises the DC of all fire-based spells cast by the weilder by 2, and can cast the following spells at 10th caster level*:
burning hands (5d4, DC 16) (1 charge)
fireball (10d6, DC 19) (1 charge)
wall of fire (DC 21) (2 charges)
*The +2 to fire spell DCs are included in the numbers.

Enough of the really expensive stuff. Here's some new basic items, stuff most people can start off with.
Bardiche: Also known as a Lochaber Axe, a bardiche originates from a halberd. It's blade head size has increased dramatically, preemting the spear tip. It's handle is lengthened, making it a deadly weapon from short distances. I can be used up to 10 ft. away, but not against adjacent opponents.

Socketed Items: Weapons can have 1 socket per size category above diminutive. Armor can have 1 socket per size category (light, medium, heavy). Shields have 1 socket for bucklers and small shields, 2 for large, and 3 for tower. A found item that can hold sockets has a 10% chance to have 1-max# of sockets when found.
Gems: Gems add damage to weapons, abilities to armor, and resistance to shields. Some gems add other than damage.

Double Edit: Gems can be removed, but doing so lowers their type by 1. Gems can be upgraded as weapons and armor can.

Heres a custom item I made but haven't had the chance to use.
The Slanted Eye
The Slanted Eye looks like a eye shape turned sideways, with a cat eye like slit running from the top to bottem. This item takes up the third eye slot of a charecter. This eye provides a mirror image spell as a free action once per round as a level 12th level wizard. You can use this item again while the first effect is up, but you must use the new roll for numbers of mirror images even if it is less.
Cost: I forget, someone can work out the price for a quickened mirror image at will item though.

According to the PsiHB, a third eye is fixed to the middle of the forhead. This would take the head space, and cost the lesser of the two.

Actually no, if you look at the wording its pretty much saying it is a new slot for a item to be worn, it does not mention a third eye taking up a slot, but it stipulates only one third eye can be worn at a time. The simple conclusion? Third eye is a new slot

If it takes up a space not mentioned in the PHB or DMG, it's not an item space. Look at Ioun Stones, they take their own spot, and you can only have one, but the spot isn't usual, and is considered as "not taking a space". Therefore, a third eye would be the higher price for the imaginary other space.

One, look at the pricing of the powers, its a sloted item
Two, unless I missed it no where does it say that you can only have Ioun stone, in fact you could have hundreds of them circling your head if you really wanted, you could even get enough to get the DM to give you a concealment bonus ;)
The point is the Psionics handbook introduces two new slots for the campaign, the skin slots (you get 3) and the third eye slot. It does not matter if its not mentioned in the DMG.

Its not adding a slot until everything is cheap, they are new slots based of a more psionic feel.
No supliments add on slots, and since the psionic handbook can almost be considered 'core rule' then if it allows 2 new slots then 2 new slots there are.

Though I allow psions in my games, I consider the book more of an overpriced PHB supplement. In my games, the Third Eye takes up the headband/hat/helmet space, and Skins take the vest/vestment/shirt spot. Without the PsiHB, there are already 13 spaces for normally-priced magic items.

Plague Bearer:
A cousin to the Hellplague, Plague Bearer is even more powerful, and has only been granted use to Balors and Pit Fiends since it's creation. It acts as a +4 longsword of venom, except that whenever it strikes, both a poison spell and a contagion spell (Slimy doom) are inflicted upon the victim. The Fortitude save for both of these effects is 20.

Lucky Break - the sword of nine lives.
A shortsword of the finest quality, Lucky Break was created out of desperation by a powerful cleric whose nephew, a particularly hapless warrior, was brought back to him time and time again in various states of serious injury or death. Each time upon being raised or healed, however, he would ignore his uncle's advice and charge recklessly back out on some perilous quest, only to return once more a bloodied corpse.
Eventually, tired of the young fighter's stubborn stupidity, the old priest raised him one last time and sent him on his way, warning his nephew that from now on, he was on his own. Knowing the young man's impetuous nature would eventually get the better of him, and caring for him deeply, the cleric gifted him with this sword, hoping its magic would allow the reckless warrior to live long enough to learn caution. Alas, his hope was in vain, and his nephew perished after insulting a master swordsman, the blade twisted from his hands by his opponent's skill before he was run through.
The sword's unremarkable blade is set in a hilt of jet-black ebony, hard as steel and cool to the touch. Eight pearls of the highest quality decorate the ornate crosspiece, four either side, a ninth and far larger gem acting as the sword's pommelstone. On closer inspection, the stones are revealed to be... not pearls, but spherical gems containing a luminescent, milky substance, glowing gently as it swirls.
Lucky Break is a +2 shortsword with the following properties. Whenever the bearer is reduced to 0hp or less, the sword automatically returns him to 10hp (or his maximum, whichever is least).
Similarly, should the bearer fail a saving throw which results in his or her death by any means other than hit-point loss, they may retake the saving throw. The new result stands, whether better or worse than before.
It will be two hours before either power functions again after use, and in that time the weapon acts as a simple masterwork shortsword. Each power uses up one charge.
The sword always comes with nine charges, and each time a charge is used up, the milky glow disappears from one of the pearls, leaving a sphere of clear glass. The pommelstone is the last to fade, and once it has done so, the sword loses its magical bonuses, becoming merely a masterwork shortsword.

taken from "The Isle of Dread" I believe..It can be worn as a ring until needed. When taken off the finger it becomes a dagger with a dweomer of +1 to +3..the ring becomes the pommel..
